Social Worker

Start Date: 5/19/2025
Pay Rate: $1800.00 - $2600.00

Position Summary – Social Worker

Position Duties

• Provides coverage as needed at any of our facilities.
• Performs comprehensive psycho-social assessments, interventions, management, support, and evaluates for discharge planning and aftercare needs of the at-risk patient population and their families.
• Addresses social determinants of health and collaborates with the multidisciplinary team.

Minimum Required Qualifications

- LCSW or LMSW (must have NY license prior to start)
- BLS certification
- Post-master's experience, preferably in a hospital or healthcare environment.

Preferred Qualifications

- 3 years in a hospital and/or healthcare setting

Length of Assignment

13 weeks

Shift / Hours per Week

8:30 am - 5:00 pm with occasional on-call/holiday coverage on rotation
