Job Summary

The Social Worker provides therapeutic intervention and support to patients and their families in an inpatient hospital setting. This role ensures continuity of care, facilitates safe discharge planning, and connects patients to appropriate community and post-hospital resources. The Social Worker serves as an advocate, resource, and integral member of the multidisciplinary healthcare team.

Responsibilities

  • Conduct psychosocial assessments of patients and families, identifying needs and planning interventions to support improved functioning.
  • Interview patients, families, and significant others to assess needs related to medical and social determinants of health.
  • Participate in multidisciplinary rounds, providing psychosocial context to the care plan.
  • Serve as a resource and advocate for patients/families regarding advanced care planning.
  • Provide preventive, protective, and supportive services to patients during adjustment to illness and life changes.
  • Collaborate with healthcare teams on in-hospital needs and post-discharge planning.
  • Maintain up-to-date knowledge of community resources, programs, and entitlements; provide education to patients and families.
  • Coordinate psychosocial discharge planning, including referrals to long-term care, sub-acute rehab/SNF, LTAC, dialysis, hospice, and housing resources.
  • Document all assessments, interventions, and discharge plans in compliance with hospital, state, and federal regulations.
  • Develop and maintain relationships with community agencies to support patient care continuity.
  • Evaluate the effectiveness of psychosocial services and participate in departmental performance improvement initiatives.
  • Uphold confidentiality and professional standards at all times.
  • Strong communication, negotiation, and conflict resolution skills.
  • Knowledge of medical social work practices, regulatory standards, and healthcare informatics.
  • Ability to implement rapid cycle change and contribute to clinical performance improvement.
  • Excellent organizational skills with the ability to manage multiple complex cases simultaneously.

Education & Experience

  • Master’s degree in Social Work (MSW) from an accredited program.
  • Current New Jersey Social Work License required.
  • Previous inpatient hospital experience strongly preferred, with emphasis on discharge planning and care coordination.

Physical Demands

This position requires standing, walking, handling/lifting, manual dexterity, and eye-hand coordination. Normal visual acuity and hearing are required. Reasonable accommodations may be made for individuals with disabilities to enable them to perform essential functions.
