CST Counselor Case Manager

to work Monday through Friday from 9 am to 5 pm. The role involves providing intensive community-based case management services for individuals with serious mental illness to promote community functioning and reduce psychiatric hospitalizations and crisis episodes. Some individuals may be part of the Colbert or Williams Consent Decrees. Benefits and Perks

Paid training for growth and success. Essential Functions

Provide and coordinate a continuum of treatment services. Manage a caseload, offering supportive counseling and concrete services as assigned, working as part of a treatment team. Complete all documentation promptly, adhering to Rule 140, DIIS, and the Williams and Colbert Consent Decrees. Meet financial productivity targets. Transport clients using personal or agency vehicles. Demonstrate professionalism, integrity, respect, and fairness in all interactions. Position Qualifications

High School Diploma required; Bachelor's degree in social work, counseling, or related field preferred. 1-3 years of clinical experience preferred. Experience in Crisis Intervention and De-escalation preferred. Proficiency in electronic health records required. Knowledge of accreditation and licensure standards preferred. Excellent communication, organization, and computer skills, including Microsoft Office. Background check clearance required. Valid driver’s license and reliable transportation required. Valid IL statutory minimum liability insurance coverage required.
