This includes crisis intervention, survival gear distribution, shelter and service referrals, and transportation support—all aimed at helping individuals transition to stable housing.
Working as part of a larger interagency team, the Navigator supports countywide efforts in homelessness prevention, service coordination, and housing access. The role requires balancing compassionate, client-centered outreach with professional communication and collaboration with decision-makers.

Collaborate with community partners, service providers, and the Adams County Street Outreach Network to coordinate services, respond to encampments and hotspots, and collect feedback on overlooked issues.
Use evidence-based approaches such as trauma-informed care, low-barrier services, harm reduction, de-escalation, and Housing First to engage and support residents.
Advocate for both individual needs and broader systems-level changes that benefit people experiencing homelessness.
Enter and maintain accurate case notes and data in the Homelessness Management Information System (HMIS); support broader outreach initiatives, proposals, and projects as needed.
