Acclaim Rehab and Nursing Center is seeking an experienced and driven
Admissions Director
to lead our admissions department. This role requires a highly organized and proactive professional who can effectively manage the referral process, coordinate with interdisciplinary teams, and ensure a seamless experience for residents and families. Key Responsibilities
Monitor and manage referrals daily via fax, email, and systems such as Allscripts, Navihealth, EPIC, and E-faxes, ensuring timely follow-up (90% within 1530 minutes).
Identify and troubleshoot admission-related operational issues, reporting findings to facility and corporate leadership.
Maintain accurate AM/PM census updates and distribute to leadership, billing, and interdisciplinary teams.
Update census data in Future Care PTS and CensusPro.
Collaborate with Billing, MDS, Concierge, Rehab, and Medicaid teams to streamline admissions.
Conduct facility tours, provide admissions packets, and ensure timely completion of documentation.
Partner with the Marketing team by attending events, strengthening community relationships, and maximizing referral opportunities.
Manage authorizations and case management for short-term HMO admissions in collaboration with UR teams.
Participate in daily stand-ups and quarterly QA/UR/hospital meetings as required.
Coach and support the admissions/marketing team while maintaining a high-paced workflow.
Qualifications
Minimum of 2 years experience in healthcare admissions (SNF or related setting); director-level experience strongly preferred.
High school diploma or GED required; Bachelors degree preferred.
Strong knowledge of Medicare/Medicaid, PDPM, RUGs II & III, managed care authorizations, and CMS/DOH regulations.
Proven leadership and communication skills with the ability to collaborate across interdisciplinary teams.
Proficiency in Microsoft Office and EMR systems; comfortable using communication tools such as WhatsApp.
Detail-oriented with strong organizational, documentation, and multitasking abilities.
