Join our dedicated team at Albany Medical Center as a Bariatric Patient Care Navigator. In this vital role, you will work closely with physicians and advanced practice providers to deliver exceptional care and support to patients navigating their bariatric journey. Position Overview: As a Bariatric Patient Care Navigator, you will provide direct patient care, triage, and assessment, while also planning, directing, and evaluating individualized care plans. You will collaborate within a multi-specialty team, contributing to improved patient outcomes and quality metrics. Your Responsibilities Include: Accurately obtaining and recording vital signs, reporting any abnormal values to the treating provider. Gathering and documenting comprehensive patient histories and focused physical assessments related to clinic visits. Administering medications and treatments under the supervision of a physician or advanced practice provider. Assisting in office procedures and documenting results in the Electronic Health Record (EHR). Managing incoming test results, patient requests, and pharmacy renewals, and facilitating necessary follow-ups. Utilizing resources effectively to achieve patient outcomes and support facility quality metrics. Delegating tasks as appropriate to licensed practical nurses (LPNs) and medical assistants (MAs). Complying with regulatory standards and internal policies to ensure quality care. Providing orientation and education to newly hired staff members. Engaging in continuous education to stay current in clinic specialty practices. Delivering health education to patients regarding their visits and overall wellness. Minimum Qualifications: Associate's degree from a state-accredited Registered Nursing program. Current New York State RN licensure. Current CPR certification. Preferred: Certification in specialty or ambulatory setting. Demonstrated excellent customer service and communication skills. Ability to effectively communicate with physicians, patients, and families. Proficient in MS Word and Excel; experience with electronic health record systems is a plus. Capable of administering medications via all routes and calculating medication doses.
