Join Albany Medical Center as a Bariatric Nursing Navigator, providing essential support and care for patients undergoing bariatric surgery. In this dynamic role, you will work with a team of multi-specialty professionals to ensure patients receive top-notch care tailored to their individual needs. Key Responsibilities: Monitor and record patient vital signs, promptly reporting abnormal values to the treating physician. Gather and document comprehensive patient histories and conduct focused physical assessments as per clinic protocols. Administer medications and treatments following protocols set by a physician or Advanced Practice Provider (APP). Assist in in-office procedures related to bariatric specialty, ensuring detailed results are documented in the Electronic Health Record (EHR). Review and triage incoming test results, patient requests, and pharmacy renewals, facilitating necessary follow-ups. Leverage resources to meet patient outcomes and support quality data metrics at Albany Medical Center, New York State, and national levels. Supervise and delegate tasks appropriately to Licensed Practical Nurses (LPNs) and Medical Assistants (MAs). Provide orientation and ongoing education to newly hired staff members. Stay current with continuous education related to clinic specialty, enhancing your knowledge base on best practices. Educate patients on their health and wellness in relation to their clinic visit. Qualifications: Associate's degree from an accredited Registered Nursing program is required. Current New York State RN licensure is mandatory. Current CPR certification is required. Certification in a specialty, preferably within an ambulatory setting, is a plus. Exceptional customer service and communication skills are essential. Able to effectively present information and answer questions from physicians, patients, and families. Proficient with MS Word, Excel, and preferably with electronic health records.
