Albany Medical Center is seeking a skilled and dedicated Bariatric Care Navigator to join our HBD - Surgery Bariatric team. In this role, you'll play a critical part in providing exceptional care to our patients, guiding them through their healthcare journey.
Work Shift:
Day (United States of America) Salary Range:
$83,200.00 - $128,960.00 Under the direction of the practice physician and/or advanced practice provider (APP), you will be responsible for: Obtaining and accurately recording patients' vital signs, promptly reporting abnormal values to the treating provider. Collecting and documenting a comprehensive patient history (Medical, Surgical, and Social) along with a focused physical assessment as per clinic protocols. Administering medications and treatments to patients in the clinic setting under the guidance of a physician or APP. Participating in in-office procedures related to clinic specialty, ensuring proper documentation in the Electronic Health Record (EHR). Reviewing and triaging incoming test results, patient requests, and pharmacy renewals while facilitating necessary call-backs to patients. Utilizing resources effectively to meet patient outcomes and contribute to quality metrics set by Facility (AMC), New York State, and National standards. Delegating appropriate tasks based on the licensure of LPN and/or MA. Adhering to AMC's compliance policies and regulatory requirements, including those set by The Joint Commission. Providing orientation and education to newly hired staff members. Engaging in continuous education related to clinic specialty to stay updated on current practices. Educating patients regarding their visits and promoting overall health and wellness. Minimum Qualifications: Associate's degree from a state-accredited Registered Nursing program. Current New York State RN licensure to practice professional nursing. Current CPR certification. Certification in specialty or ambulatory setting is preferred. Excellent customer service and communication skills. Adept at effectively presenting information and responding to inquiries from physicians, patients, their families, and colleagues. Proficiency in computer software, particularly MS Word and Excel. Experience with electronic health records is preferred.
