The Social Work Care Manager (CM) works with physician practices and inpatient teams to promote effective utilization of services and coordination of patient-centered care. The CM reviews plans of care, identifies barriers to utilization of resources, and assesses patients’ psychosocial, financial and clinical needs throughout the continuum of care. The CM serves as a liaison between patients, physician practices, the hospital, and the community to ensure continuity of care and psychosocial support. Current license in Illinois as Social Worker (LSW or LCSW). Ability to perform all job components and serve as a team resource for socially or clinically complex cases within their professional expertise. (Social Work) Three years’ experience as a health care provider for neonate, pediatric, adolescent, adult and/or geriatric patients, with expert knowledge in case management, discharge planning, social services, and criteria application. Ability to perform tasks independently, prioritize workload, and analyze data. Experience with information systems and Windows technology; competency within the UM Information System. Team-building skills and flexibility. Preferred Job Qualifications
Master’s in Social Work from an accredited university. Physical Demands
Role requires walking and climbing stairs throughout the Medical Center. Responsibilities
Functions as an expert clinical practitioner within the Case Management team and serves as a resource to assigned physician practice groups. Shares professional expertise as a social worker and supports CMs in managing socially complex cases. Provides basic assessment of patients’ health care needs, monitors progress, and communicates variances from the anticipated plan of care with the physician practice group and health care team. Seeks assistance as needed for complex clinical care issues. Provides and coordinates social services to patients and their families. Assesses patients’ psychosocial, financial, cultural, and family situations. Manages a complex patient caseload and coordinates discharge management services and continuity of care planning. Identifies community resources and maintains contact with them; consults with physicians and inpatient groups regarding the use of community resources. Reviews admissions and ongoing stays for medical necessity, appropriateness, and quality of care. Applies criteria, performs concurrent reviews, and works with physicians and Medical Director on case issues and denials as indicated. Applies pathways and guidelines, assesses variances, and proposes interventions. Participates in development, implementation, and evaluation of pathways/guidelines and process improvement plans. Collaborates with inpatient staff and physician groups to determine goals for length of stay management and discharge planning; promotes cost-effective, quality care services. Monitors and reports potential quality issues and makes recommendations when appropriate. Utilizes the CM information system and other technologies. Monitors variances and trends related to select goals and assists teams to develop action plans for improvement. Promotes customer satisfaction among patients, families, physicians, external case managers, payors, vendors, and inpatient staff. Represents ICARE principles in daily practice. Serves on hospital-wide and departmental committees when applicable. Employment Information
