Meets directly with patient/family to perform a comprehensive psychosocial assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with an interdisciplinary assessment of the patient. Recommends a plan of intervention based on mutually established goals. Conducts and documents initial psychosocial assessment, appropriate interventions, and expected transition in a comprehensive, clear, timely and legible manner, addressing: reactions to illness, adjustment to medical setting and treatment plan, post-hospital/clinic care needs, and linkage to community resources; facilitates surrogacy/guardianship when needed and leads efforts to identify a surrogate decision maker. Coordinates groups for supportive interventions or educational opportunities. Addresses financial issues related to insurance coverage and payment; refers to Financial Case Management Unit. Manages conflict resolution and complex family dynamics impacting plan of care and discharge planning; coordinates complex clinical case conferences. Investigates, manages, and reports suspected abuse or neglect of minors and vulnerable adults; facilitates involvement of IDCFS, IL Department on Aging, and, when necessary, Chicago Police Department. Assesses for depression and suicidal ideation using validated tools; addresses psychiatric symptoms and substance use disorders. Provides supportive care for hospice/palliative care considerations and participates in family meetings to discuss goals of care and end of life planning. Assesses needs of unhoused patients and evaluates physical environment and support systems to prevent crises or hospitalization. Participates in quality improvement, cross-coverage, training, data collection, and performance improvement activities as a social work representative. Lead groups or education sessions as needed; maintain competency for LSW/LCSW and credentialing to enable revenue generation for services. Inpatient and ambulatory medical social work; manages discharge planning through placement coordination and resource utilization; participates in daily rounds and ensures coordinated communication of the plan of care. Escalates barriers to supervisor/director and works to resolve delays; identifies and resolves barriers to timely discharge; collaborates with care teams for transitions of care to various post-acute settings. Maintains knowledge of community resources and advocates for patients to access needed information and services. Transplant program responsibilities include evaluating candidates, guiding through transplant phases, coordinating with inpatient social workers, and pursuing credentialing as applicable. Maternal & Child Health, Oncology, Behavioral Health, Bariatrics responsibilities as listed, including psychosocial assessments, care planning, and interdisciplinary coordination. On-Call responsibilities: provide phone consults and coordinate discharge planning after hours; participates in disaster response as needed. Minimum Qualifications
Master's degree from an accredited school or university in social or behavioral science or a related health specialty area. Current State of Illinois Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW). Preferred Qualifications
