Description
Join our dedicated Home Health team as an Occupational Therapist and make a meaningful impact by helping patients regain independence and improve their quality of life in their own homes. Provide personalized, patient-centered occupational therapy services across a beautiful and diverse service area from Ninilchik to Kachemak Selo, working closely with a collaborative healthcare team.

RESPONSIBILITIES:
Evaluate patients in their homes and develop individualized occupational therapy treatment plans. Provide skilled occupational therapy services according to physician orders and best practice standards. Direct and supervise assigned personnel, including CNAs, as needed. Communicate and collaborate effectively with patients, families, medical staff, and the Home Health team. Maintain thorough documentation including initial evaluations, progress notes, treatment plans, and discharge summaries.
KNOWLEDGE AND EXPERIENCE:
Graduate of an accredited Occupational Therapy program and successfully completed or eligible for the National Certification Examination. Licensed to practice as an Occupational Therapist in the state of Alaska. Two years of clinical experience preferred. Current Basic Life Support (BLS) certification required and maintained. Strong communication skills to interact with patients, families, and healthcare professionals. Knowledge of occupational therapy practices in home health or community settings. Ability to perform evaluations, develop care plans, and document patient progress effectively. Physical requirement: Ability to walk, stand for extended periods, stoop, and lift up to 50 pounds.
ENVIRONMENT:
South Peninsula Hospitals Home Health Services operate within a 22-bed critical access hospital and 28-bed long-term care facility located in the scenic Kenai Peninsula. This role requires travel throughout the service area, which includes inclement weather conditions. Most work is performed inside patient homes with moderate physical demands such as walking, standing, and lifting. This position involves exposure to infectious diseases, blood-borne pathogens, muscle strains, and the risk of falls during home visits and clinical work.
