RESPONSIBILITIES:

Evaluate patients and develop individualized rehabilitation plans based on physical, psychosocial, and functional needs. Deliver direct physical therapy services in patients' homes in accordance with physician orders and best practices. Monitor, document, and adjust treatment plans based on ongoing assessments and patient responses. Educate patients and families on mobility, safety, and self-care using clear communication and teach-back methods. Collaborate with the interdisciplinary Home-Based Health Services (HBHS) team to ensure holistic patient care and seamless service delivery.

KNOWLEDGE AND EXPERIENCE:

Current license to practice physical therapy in the State of Alaska. Current Basic Life Support (BLS) certification required. Minimum one year of experience in Home-Based Health Services preferred (or willingness to undergo structured training). Strong working knowledge of OASIS documentation and Medicare/Medicaid regulations. Proficient in clinical assessments, restorative techniques, and individualized treatment planning. Effective verbal and written communication skills for patient care documentation and interdisciplinary collaboration. Familiarity with electronic health record systems (e.g., Kinnser) is a plus. Physical requirement: Ability to walk, stand for extended periods, stoop, and lift up to 50 pounds.

ENVIRONMENT:

This position offers a blend of independence and collaboration, providing in-home care throughout a rural, coastal service area from Ninilchik to Kachemak Selo. Therapists often work autonomously and may travel in varying weather conditions to reach clients. Though most duties occur indoors, the role requires regular travel and physical mobility. Exposure to infectious diseases, environmental hazards, and physical strain is possible, especially when assisting clients with limited mobility in home settings.
