Position Overview
The Social Worker MSW collaborates effectively with healthcare teams to ensure patients receive safe, timely services and families are equipped with necessary resources and education. You will assess psychosocial risks, conduct mental health evaluations upon physician referral, and facilitate complex discharge planning. Additionally, you will serve as a key contact for cases involving abuse, neglect, guardianship, foster care, and mental health placements, acting as a liaison with protective services and community agencies.
Key Responsibilities
- Collaborate with multidisciplinary teams to facilitate safe, effective, and timely patient care.
- Interpret and communicate complex patient and family needs, modeling caring and professional practices.
- Assess psychosocial risk factors by evaluating prior functioning, support systems, reactions to illness, and coping abilities.
- Counsel patients and families regarding emotional, social, and financial impacts of illness or disability.
- Access and mobilize community and family resources to meet patient needs.
- Conduct mental health assessments upon physician referral and provide therapeutic interventions.
- Support complex discharge planning, including guardianship, foster care, adoption, and mental health placements.
- Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
- Liaise with protective services, guardianship, foster care, and mental health agencies.
- Utilize crisis intervention and de-escalation techniques to support patients in acute situations.
- Develop and maintain strong relationships with community agencies, law enforcement, schools, and other stakeholders.
Qualifications & Requirements
- Education: Master’s Degree in Social Work (MSW) required.
- Licensure: Permanent LSW licensure required; LCSW licensure preferred.
- Experience: 0-3 years of relevant healthcare, mental health, or community-based experience required.
- Certifications: De-escalation certification preferred.
- Skills & Knowledge:
- Knowledge of disease impact and human behavior for psychosocial assessments and crisis intervention.
- Understanding of state and county legal mandates related to child and adult protective services.
- Principles of growth and development across the lifespan.
- Strong crisis de-escalation and conflict resolution skills.
- Ability to develop and maintain community and healthcare partnerships.
- Proficiency in Microsoft Excel and Word.
