Position Summary:
The Patient Navigator plays a vital role in supporting patients receiving addiction treatment by helping them overcome barriers to care. This position serves as a liaison between patients and providers, ensuring coordinated access to services such as transportation, housing, medication, and health benefits.
Key Responsibilities:
Assist patients with transportation arrangements to and from appointments.
Support patients in refilling medications and managing prescriptions.
Facilitate treatment and medication authorizations with insurance providers.
Coordinate appointments across multiple providers and services.
Help patients navigate housing resources and referrals.
Assist with health benefits coordination, including Medicaid/Medicare enrollment and insurance questions.
Act as a liaison between patients and healthcare providers, ensuring clear communication and follow-up.
Maintain accurate documentation of patient interactions and services provided.
Collaborate with clinical teams to identify patient needs and develop care plans.
Provide compassionate, nonjudgmental support to individuals in recovery.
Qualifications:
High school diploma or equivalent required; associate or bachelor’s degree preferred.
Experience in healthcare, social work, or addiction services strongly preferred.
5 years Lived experience with addiction (through work or personal experience)
Knowledge of community resources, insurance systems, and behavioral health services.
Strong communication, organizational, and problem-solving skills.
Ability to work independently and as part of a multidisciplinary team.
Working Conditions:
May require travel between sites
Office and clinical settings
Salary/Benefits:
In compliance with The State of New Jersey's Pay Transparency Act, the annual base salary range for this position is $48,509.15-$60,000, with placement within the range determined by candidate's qualifications, professional experience, internal equity, and other factors."
For eligible employees, benefits include: comprehensive NJ State health, dental and retirement benefits, extensive accrued time off/paid holidays, and tuition reimbursement for employee, spouse, and dependents to attend Rowan University, Glassboro. (Tuition reimbursement will apply to undergraduate degrees only for spouse and dependents).”
