Covenant Case Management Services is seeking an enthusiastic Physical Therapist to join our team!
The SCS Physical Therapist provides specialized consultative services to individuals with Intellectual and Developmental Disabilities (IDD) enrolled in the NC Innovations Waiver. The PT delivers person-centered consultation, evaluation, and training designed to promote independence, safety, and improved quality of life. This is a consultative role, not direct hands-on therapy, with a focus on training staff, caregivers, and support teams in implementing strategies within daily routines.
Key Responsibilities Conduct initial and ongoing assessments of members’ mobility, positioning, transfers, and adaptive equipment needs. Develop and update individualized consultative plans aligned with the member’s Person-Centered Plan (PCP). Provide training and technical assistance to Direct Support Professionals (DSPs), Qualified Professionals (QPs), and caregivers on safe and effective implementation of recommendations. Monitor and review progress, ensuring strategies are integrated consistently across environments (home, day program, community). Document all consultative activities in compliance with NC Innovations Waiver, MCO, and agency requirements. Collaborate with interdisciplinary team members (nursing, occupational therapy, speech therapy, behavior specialists, case managers, etc.) to coordinate holistic support. Assist with the evaluation and recommendations for durable medical equipment (DME), adaptive seating, positioning systems, and mobility devices as appropriate. Participate in treatment team meetings, intake committee reviews, and care planning discussions as needed. Maintain knowledge of NC Medicaid, Innovations Waiver requirements, and current best practices in IDD and physical therapy. Equal Opportunity Employer:
