Physical Therapist – LTC/SNF - Travel Position Location: Snow Hill, NC Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Days Pay: $2,035/week
Job Description: We are seeking a dedicated and compassionate Physical Therapist to join a skilled nursing and long-term care facility in Snow Hill, NC, on a travel contract. The Physical Therapist will evaluate residents, design individualized treatment programs, and provide therapy services that promote mobility, restore function, and enhance independence. The ideal candidate is patient-focused, detail-oriented, and experienced in geriatric rehabilitation.
Key Responsibilities: Conduct comprehensive evaluations to determine mobility, strength, and therapy needs. Develop and implement individualized treatment plans in coordination with physicians and care teams. Deliver therapeutic interventions including strengthening, gait training, balance exercises, and mobility support. Educate residents, families, and caregivers on therapy goals, safe practices, and assistive equipment. Document evaluations, progress notes, and treatment outcomes accurately and timely. Collaborate with interdisciplinary staff to ensure coordinated, patient-centered care. Promote safety, independence, and quality of life through evidence-based therapy services.
Qualifications: Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program. Current and valid Physical Therapist license in North Carolina (or compact license). Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care. Current certification in Basic Life Support. COVID-19 vaccine and recent tuberculosis test. Strong communication, assessment, and organizational skills. Compassionate approach with experience supporting geriatric populations.
