Social Worker – Full Time, Days

What You’ll Do:

• Complete psychosocial assessments and develop personalized care plans

• Educate patients and families on advance directives and alternate levels of care

• Coordinate discharge planning and connect patients/families with community resources

• Complete MDS 3.0 assessments and contribute to care team meetings

Qualifications

• Bachelor’s Degree in Social Work

• Experience in long-term care or sub-acute/rehab setting preferred

• Knowledge of discharge planning, community resources & MDS 3.0

• Strong communication, organizational, and interpersonal skills

• Compassion and commitment to serving the elderly population
