Overview

DaVita is looking for a Licensed Social Worker (Hybrid) to work with our dialysis patients. You will not be assigned to a specific location and you must be willing to get licensed in multiple states (DaVita will reimburse the cost).

Responsibilities

• Schedule: Monday – Friday 9:00am – 5:00pm (some flexibility).
• Social workers are responsible for the psychosocial health and wellbeing of our patients, providing interventions as needed and completing mandatory assessments, screenings, and education.
• Typically manage a caseload of around 115–125 patients, spread across clinics, which may vary.
• Build meaningful, long-term relationships with patients and their families in an intimate outpatient setting.

In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. You will work independently with substantial support on the social work side.

Requirements

• Willing to obtain multi-state licenses (reimbursed).
• Meets all state requirements to practice in the dialysis setting.
• All social workers must hold an LSW licensure.
• Master’s degree in Social Work (MSW) required with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, and functional proficiency with DaVita-specific applications within 60 days.

Compensation and Benefits

• Salary: $27.00 - $49.50 per hour (location-specific minimum wage details available via DaVita resources).
