Position description includes essential functions but not limited to listed duties. Responsibilities span administrative leadership, nursing care, committee participation, staff development, safety, equipment, care planning, budgeting, and supervisory responsibilities in a long-term care facility.

Responsibilities

  • Administrative Functions — Assist the Director of Nursing Services in directing day-to-day nursing activities in accordance with current rules, regulations, and guidelines; develop, maintain, and update written policies and procedures for the nursing service department; ensure reference materials (PDRs, regulations, standards) are current; recommend materials to meet residents’ needs; maintain a current Nursing Service Procedures Manual; ensure compliance with procedures; provide written and oral reports to the Director; periodically review policies, procedure manuals, job descriptions, and make revisions as needed; coordinate nursing services with other resident services to ensure continuity of care; ensure personnel are in compliance with job descriptions; participate in the facility\'s quality assurance program; participate in facility surveys as requested; review and update discharge plans; plan nursing services for discharge; meet with staff to plan services and activities; admit, transfer, and discharge residents; complete accident/incident reports; interpret policies to personnel, residents, visitors, and government agencies; perform administrative duties (forms, reports, evaluations, studies, charting); protect resident information and report violations; report unauthorized access attempts.
  • Committee Functions — Attend committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assurance) as required; prepare reports of nursing programs and activities; provide information to the Quality Assurance and Assessment Committee; participate in Care Plan Committee agenda development and discharge plan reviews; implement committee recommendations as instructed.
  • Personnel Functions — Ensure adequate staffing of licensed nurses and certified nursing assistants; develop and assign work; delegate administrative authority to Charge Nurse; ensure all aides are properly trained; perform daily rounds to verify standards; monitor absenteeism; report problem areas; enforce policies and dress codes; review personnel complaints and grievances; foster interdepartmental collaboration to meet resident needs; maintain a warm, calm environment; meet with shifts to identify problem areas and improve services; evaluate staff and participate in performance evaluations; determine staffing and potential dismissals/transfers; administer disciplinary actions fairly; report occupational exposures, fraud, and security concerns; ensure workstation security and password protection according to policy.
  • Nursing Care Functions — Inform nursing staff of admissions, room assignments, and readiness of rooms; greet and escort new admissions; rounds with physicians as needed; encourage timely physician notes and orders; plan resident care with residents, families, and physicians; notify changes in condition to physician and next of kin; perform daily resident rounds and ensure licensed staff provide direct care; review and ensure accuracy of nurses\' notes; start IVs and collect specimens as ordered; perform vital signs and routine measurements; admit/transfer/discharge residents as needed; monitor residents and corrective actions; authorize restraints per policy; ensure medication administration and storage procedures; inspect treatment areas; ensure call checks for residents; coordinate transportation for discharge; inform families of deaths; contact funeral homes as requested; report problems to the Director; assist with discharge planning and improvement of services.
  • Staff Development — Develop and schedule in-service training; assist in annual facility training programs (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control); plan clinical supervision for nurse aide trainees; participate in continuing education to maintain licensure.
  • Safety and Sanitation — Develop safety standards; ensure exposure control training; provide PPE; enforce hand hygiene; conduct fire/disaster drills; maintain infection control program for communicable diseases and isolation precautions; develop procedures for reporting hazards; ensure safe operation and training for equipment; ensure MSDS documentation; promote safe lifting and equipment use; report missing/illegible labels.
  • Equipment and Supply Functions — Advise on equipment and supply needs; maintain adequate stock of medications and supplies; assist in preventive maintenance; develop safe operation procedures for equipment; ensure trained personnel operate equipment; monitor supply usage to prevent waste; ensure hazardous chemical documentation is on file.
  • Care Plan and Assessment Functions — Participate in preliminary/comprehensive resident assessments; develop and review care plans with resident involvement; ensure staff are aware of care plans; refer to care plans before care delivery; review progress notes against care plans; assist in planning and revising the MDS and RAPs; ensure treatments align with care plans and resident wishes.
  • Budget and Planning Functions — Monitor economic conditions and advise on nursing service adjustments; recommend equipment needs; report suspected fraud.
  • Supervisory Requirements — Contribute to supervision/management of nursing staff; carry administrative authority for assigned duties as an RN.

Qualifications

  • Education and Experience — Nursing degree from an accredited college/university; training in rehabilitative/restorative nursing; 2+ years experience, preferably in a long-term care facility.
  • Language Skills — Able to read, write, speak, and understand English; ability to read technical procedures and manuals; able to present information to managers and employees.
  • Mathematical/Reasoning — Ability to apply fractions, percentages, ratios; independent decision-making; problem-solving; tactful communication; leadership and supervisory ability; ability to plan, organize, implement, and interpret programs for quality care; knowledge of age-appropriate resident care; teamwork; knowledge of nursing practices, regulations, and guidelines; patience, tact, and enthusiasm; willingness to adopt new practices; ability to convey resident condition information; must not pose a direct health/safety threat; ability to relate to residents and staff; be able to work with ill or upset individuals.
  • Licenses/Certifications — Current, active RN license; current CPR certification; PCC knowledge preferred; maintain continuing education/licensing; good standing with State Board of Nursing.
  • Physical Demands — Ability to move intermittently, speak and write English clearly, handle mental and emotional stress, see/hear or use aids as needed, work independently with integrity and flexibility; meet health requirements; relate to residents and staff; lift at least 25 pounds to 5 feet and move 50 feet; may assist in evacuations.
  • Work Environment — Office and nursing areas; frequent interruptions; exposure to residents, staff, and outside parties; possible hostile/upset individuals; may require extended hours, weekends, and holidays; may involve emergency back calls; exposure to hazards and contaminants; participate in continuing education; potential exposure to infectious waste and chemicals.
