Physical Therapist – LTC/SNF – Travel Position
Location: Dallas, TX Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Days Pay: $2,145/week
Job Description
We are seeking a compassionate Physical Therapist to join a long-term care/skilled nursing facility (LTC/SNF) in Dallas, TX on a 13-week travel assignment. The Physical Therapist will provide rehabilitative services to residents, focusing on mobility, strength, and functional independence in a supportive care environment. The ideal candidate has prior LTC/SNF experience and is committed to delivering high-quality, patient-centered care.
Key Responsibilities
Perform comprehensive evaluations to assess residents’ mobility, strength, and functional abilities.
Develop and implement individualized therapy plans tailored to long-term care needs.
Provide interventions including gait training, strengthening, balance, and mobility exercises.
Educate residents, families, and caregivers on therapy goals, home exercise programs, and safe mobility practices.
Document therapy sessions, care plans, and progress notes accurately in compliance with facility and regulatory standards.
Collaborate with occupational therapy, speech therapy, nursing, and interdisciplinary teams to ensure coordinated care.
Support residents’ independence, mobility, and overall well-being through evidence-based PT practices.
Qualifications
Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
Current and valid PT license in Texas (or compact eligibility).
Minimum 1 year of recent PT experience; LTC/SNF experience preferred.
Current certification in Basic Life Support (BLS).
Strong communication, documentation, and clinical reasoning skills.
Compassionate approach with experience treating geriatric and long-term care populations.
COVID-19 vaccine and recent tuberculosis test.
