Physical Therapist – LTC/SNF – Travel Position

Location: Scottsdale, AZ Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Days Pay: $1,895/week

Job Description

We are seeking a compassionate Physical Therapist to join a long-term care/skilled nursing facility (LTC/SNF) in Scottsdale, AZ on a 13-week travel assignment. The Physical Therapist will help residents restore mobility, improve strength, and enhance overall quality of life within a supportive care environment. The ideal candidate has prior LTC/SNF experience and is committed to providing patient-centered rehabilitative care.

Key Responsibilities

Perform comprehensive physical therapy evaluations for residents in a long-term care/skilled nursing setting.

Develop and implement individualized treatment plans tailored to residents’ needs.

Provide therapeutic interventions such as gait training, strengthening, mobility exercises, and balance training.

Educate residents, families, and caregivers on safe mobility strategies and exercise programs.

Document evaluations, treatment sessions, and progress notes accurately and in compliance with facility and regulatory standards.

Collaborate with occupational therapy, speech therapy, nursing, and interdisciplinary teams to ensure coordinated care.

Promote independence, mobility, and overall well-being through evidence-based PT practices.

Qualifications

Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.

Current and valid PT license in Arizona (or compact eligibility).

Minimum 1 year of recent PT experience; LTC/SNF experience preferred.

Current certification in Basic Life Support (BLS).

Strong clinical reasoning, communication, and documentation skills.

Compassionate approach with experience treating geriatric and long-term care populations.

COVID-19 vaccine and recent tuberculosis test.
