Description Conduct evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs. Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function. Work collaboratively with a multidisciplinary team, including nurses, physicians, case managers, and rehabilitative specialists, to optimize patient outcomes. Educate patients and their families on therapy goals, discharge planning, and home exercises to foster recovery. Maintain accurate documentation of evaluations, treatments, and patient progress in alignment with facility protocols and regulatory standards. Engage in interdisciplinary discussions during care team rounds to ensure seamless patient care. Education

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited program. Certifications

Valid state licensure as a

Physical Therapist . Preferred: CPR certification. Additional certifications in specialized areas (e.g., neurological, geriatric) are advantageous but not mandatory. Skills

Prior experience is highly desirable. Expertise in treating patients with complex medical issues and mobility challenges. Exceptional communication and teamwork abilities. Proficient in digital charting and clinical documentation systems. Benefits
