Description Assess patients in either inpatient or outpatient settings and develop tailored treatment plans Implement physical therapy interventions that enhance mobility, strength, balance, and overall function Work hand-in-hand with nurses, physicians, case managers, and fellow rehab professionals to maximize patient outcomes Educate patients and their families on therapeutic objectives, discharge planning, and home exercises Maintain documentation of evaluations, treatments, and patient progress according to facility and regulatory standards Engage actively in care team discussions and interdisciplinary meetings to provide cohesive patient care Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications
Valid state license as a
Physical Therapist CPR certification preferred Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory Skills
Prior experience in a similar role is strongly preferred Ability to manage patients with intricate medical and mobility needs Exceptional communication and collaborative skills Familiarity with digital charting and clinical documentation software Benefits
