You will deliver physical therapy interventions aimed at improving mobility, strength, balance, and overall function, collaborating alongside nurses, physicians, case managers, and fellow rehab professionals to ensure optimal outcomes. Education and communication will be crucial in this role. You will have the opportunity to educate patients and their families on therapeutic goals, discharge planning, and home exercises, documenting evaluations and progress in alignment with facility guidelines and regulations. Moreover, your participation in care team rounds and interdisciplinary conferences will play a critical role in ensuring coordinated and comprehensive care. Candidates should possess a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational program, along with a valid state licensure as a Physical Therapist.
