Job Responsibilities Conduct evaluations for patients in diverse settings, both inpatient and outpatient, creating customized treatment plans. Deliver hands-on physical therapy interventions with an emphasis on mobility, strength, balance, and overall function. Work closely with an interdisciplinary team of nurses, physicians, and rehabilitation professionals to enhance patient outcomes. Guide and educate patients and their families on therapeutic objectives, discharge processes, and home exercise routines. Maintain accurate documentation of assessments, treatments, and patient progress, in line with facility and regulatory standards. Engage in multidisciplinary care team rounds and collaborative conferences to promote seamless patient care. Education Requirements
Doctor of Physical Therapy (DPT) or an equivalent degree from an accredited institution. Preferred Certifications
State license for practicing as a
Physical Therapist Current CPR certification is preferred. Advanced certifications (such as in neurological or geriatric therapy) are advantageous but not mandatory. Skills & Qualifications
Prior experience in the field is highly preferred. Capability to provide care for patients facing complex medical and mobility challenges. Excellent communication and teamwork abilities. Proficient in digital health charting and clinical documentation practices. What We Offer
