What You’ll Do
Provide professional, person-centered care with initiative and responsibility.
Collaborate with interdisciplinary teams, hospital case management, and GRHD’s quality team to ensure smooth hospital-to-Care Center transitions.
Coordinate all admission processes, including:
Screening referrals and inquiries
Completing on-site assessments and admission determinations
Managing bed assignments and preliminary paperwork
Supporting social workers with resident/family psychosocial needs
Assisting with room changes and facilitating smooth transitions from admission to discharge
Serve as liaison with hospitals and families; provide admission packets and concierge support during move-in.
Maintain compliance with standards and ensure accurate documentation for reimbursement.
Provide general financial knowledge of nursing operations and coordinate related communications.
Lead and participate in community marketing and outreach, including facility tours and special events.
Promote hospitality, customer service, and positive community relationships.
Coordinate interdisciplinary plans of care from admission to discharge, acting as liaison among patients, families, providers, and insurers.
Evaluate patient needs and resources; develop effective discharge plans and ensure continuity of care.
Identify care issues and recommend pathway improvements; may conduct utilization reviews.
Perform discharge planning tasks: meetings, physician orders, home health referrals, and equipment arrangements.
Who We’re Looking For
Bachelor’s degree in a related field of study preferred.
3 to 5 years of previous professional experience in a long term care setting is highly preferred
What We Offer Our total rewards package includes
Pay Range:
$37.94 - $41.56/hour (based on experience)
Shift Differentials:
$2–$4/hour for nights and weekends
