Join a compassionate team delivering interactive mental health services to adults and children facing significant mental health challenges in the Midwest region. This role focuses on providing a supportive, normalizing experience that enhances overall functioning while helping individuals remain in the least restrictive environment possible-their home and community.

Essential Responsibilities

Provide skill training aligned with target goals and measurable outcomes as determined by a Qualified Mental Health Professional. Deliver intensive, community-based mental health services using a strengths-based, skill-building approach. Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans. Help clients understand their symptoms and explore possible solutions. Support clients and families in engaging natural community supports to foster solutions. Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners. Adhere to privacy and security regulations regarding Protected Health Information (PHI). Maintain professional and effective communication with clients, team members, and external agencies. Education and Experience

Required: Bachelor's degree in Social Work, Psychology, or related field; or Associate's degree plus 2+ years of experience educating clients on self-care and daily living skills; or equivalent combination totaling 4+ years. Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics. Preferred experience: 3-5 years in mental health services, crisis response, or related fields. Licenses and Certifications

Required: CPST Case Management training, KHS training, background checks including KBI, child and adult abuse registries, motor vehicle screening. Completion of Adult Case Management training within 60 days of hire. Valid driver's license, insurance, and reliable vehicle if driving is required. Skills and Abilities

Familiarity with community mental health services. Strong organizational skills. Intermediate proficiency with Microsoft Office.

Physical Requirements

Light work involving occasional exertion up to 25 lbs and frequent exertion up to 10 lbs. Standing or walking may occur 11-25% of the day.
