ASW | Adult & Youth Case Manager

Join a compassionate team as an Adult & Youth Case Manager providing direct community-based mental health and applied behavior analysis services. This role focuses on supporting children and adults facing significant mental health and social challenges by delivering evidence-based interventions in home, school, or community settings.
Key Responsibilities
Implement skill training aligned with treatment goals using evidence-based practices, with measurable outcomes determined by qualified mental health professionals. Follow treatment plans developed by BCBA/Autism Specialists and actively participate in supervision and training sessions. Deliver intensive, interactive mental health and behavioral services using a strengths-based, skill-building approach. Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans. Support clients and families in understanding symptoms and exploring solutions, including leveraging natural community supports. Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners. Perform additional duties as assigned to enhance client care and service delivery. Education & Experience
Required: Bachelor's degree in Social Work, Psychology, or related field; OR Associate's degree plus 2+ years of experience educating clients on self-care and daily living skills; OR equivalent combination totaling 4+ years. Preferred: Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics. Preferred: 3-5 years of professional experience in mental health or related services. Licenses & Certifications
Completion of CPST Case Management online training, KHS online training, and passing background checks including KBI, child and adult abuse registries, and motor vehicle screening. Completion of state-required Adult Mental Health Case Management training within 60 days of hire. Completion of Intensive Individual Support training within 6 months of hire. Valid driver's license, insurance, and reliable vehicle required. Skills & Attributes
