Description Conduct patient evaluations in both inpatient and outpatient settings, customizing treatment plans to individual needs Implement therapeutic interventions that focus on enhancing mobility, strength, balance, and overall functionality Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes Provide education to patients and families on treatment objectives, discharge planning, and home exercise regimens Maintain comprehensive documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory guidelines Engage in interdisciplinary care team meetings to ensure cohesive patient care Education

Doctorate in Physical Therapy (DPT) or an equivalent accredited degree Certifications

Current state licensure as a

Physical Therapist CPR certification strongly preferred Specialty certifications such as in geriatrics or neurological conditions are advantageous but not required Skills

Experience in a clinical setting is strongly preferred Ability to treat patients with a range of complex medical conditions and mobility challenges Excellent communication and teamwork capabilities Proficient with digital documentation and electronic health records systems Benefits
