Description Conduct thorough evaluations of patients in both inpatient and outpatient settings while creating personalized treatment plans Implement physical therapy interventions concentrating on mobility, strength, balance, and functional improvement Work alongside nurses, physicians, case managers, and a diverse range of rehabilitation professionals to maximize patient outcomes Provide education for patients and their families regarding therapeutic objectives, discharge planning, and home exercise routines Document assessments, treatment progress, and outcomes in full compliance with facility standards Engage actively in care team rounds and interdisciplinary meetings to ensure coordinated, holistic patient care Education
Doctorate in Physical Therapy (DPT) or comparable degree from an accredited institution Certifications
Active state licensure as a
Physical Therapist CPR certification is preferred Specialty certifications (for instance, in geriatrics or neurology) are beneficial but not mandatory Skills
Prior experience in a similar setting is highly desirable Adept at managing patients with complex medical situations and mobility challenges Excellent communication and collaborative teamwork capabilities Proficient with electronic charting and clinical documentation tools Benefits
