Description Thoroughly evaluate patients in both inpatient and outpatient settings and develop personalized treatment plans. Implement therapeutic interventions that prioritize mobility, strength, balance, and overall functional improvement. Collaborate closely with nurses, physicians, case managers, and rehabilitation specialists to maximize patient results. Counsel patients and their families regarding therapeutic objectives, discharge processes, and at-home exercises. Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility protocols. Engage in interdisciplinary team rounds and conferences to ensure a seamless care experience. Education

Doctorate in Physical Therapy (DPT) from an accredited institution or equivalent degree. Certifications

State licensure as a

Physical Therapist . CPR certification is preferred. Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory. Skills

Prior experience is highly desired. Proficient in managing complex medical and mobility-related challenges. Excellent communication and teamwork capabilities. Familiarity with electronic charting and clinical documentation systems. Benefits
