Description Conduct patient evaluations in either inpatient or outpatient settings and create customized treatment plans Implement targeted physical therapy interventions with an emphasis on mobility, strength, balance, and overall function Work collaboratively with nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes Educate patients and their families on therapeutic goals, discharge processes, and at-home exercise regimes Maintain accurate documentation of evaluations, treatments, and patient progress per facility and regulatory requirements Engage in care team rounds and interdisciplinary meetings to ensure unified patient care Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications
Active state licensure as a
Doctor of Physical Therapy CPR certification is preferred Specialized certifications (such as neurological or geriatric) are advantageous, but not mandatory Skills
Prior experience in the field is strongly preferred Ability to care for patients with complex medical and mobility situations Exceptional communication and teamwork abilities Proficiency with digital charting and clinical documentation systems Benefits
