Description Conduct thorough evaluations of patients in both inpatient and outpatient settings and develop customized treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional capacity Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes Educate and guide patients and their families on therapeutic goals, discharge planning, and exercises for home practice Maintain precise documentation of evaluations, treatments, and progress per facility and regulatory guidelines Participate in care team rounds and interdisciplinary meetings to ensure cohesive patient care Education

Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution Certifications

Active state licensure as a

Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are welcomed but not mandatory Skills

Prior experience is highly preferred Ability to manage patients with intricate medical and mobility challenges Exceptional communication and teamwork abilities Familiarity with digital documentation and clinical systems Benefits
