Description Evaluate patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality Engage in meaningful collaborations with nurses, physicians, case managers, and other rehab professionals to maximize patient outcomes Provide education to patients and their families regarding therapeutic objectives, discharge planning, and home exercise programs Document evaluations, treatments, and patient progress meticulously in line with facility and regulatory protocols Participate actively in interdisciplinary conferences and care team rounds to ensure cohesive and coordinated care Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution Certifications
Current state licensure as a
Physical Therapist CPR certification is preferred Specialized certifications (for example, in geriatrics or neurology) are advantageous but not mandatory Skills
Prior experience is strongly preferred Aptitude for treating patients with complex medical and mobility challenges Exceptional communication and collaboration skills Proficient in digital charting and clinical documentation systems Benefits
