She Recruits has partnered with a mission-driven, high-acuity rehabilitation and long-term care center in the Bronx in search of an experienced
MDS Director
to take the lead on all things assessment, compliance, and care coordination.

Why This Role Rocks: You'll step into a leadership role with impact-managing a skilled team of 7 MDS nurses while shaping clinical and financial outcomes. You'll be the brain behind the facility's MDS strategy, ensuring accuracy, timeliness, and reimbursement optimization under PDPM and Medicaid models. You'll collaborate with a strong interdisciplinary team across nursing, therapy, social work, and dietary to ensure every resident's story is captured and supported. You'll bring clinical, regulatory, and financial awareness into alignment-and you'll do it with autonomy, support, and recognition. Your Day-to-Day: Direct the daily operations of the MDS department with full ownership of assessment quality and compliance. Lead MDS scheduling, coding accuracy, and documentation integrity to support both resident care and facility reimbursement. Analyze quality metrics and CMI data to ensure optimal performance and outcomes. Participate in QA committees and lead care plan conferences with residents, families, and clinical teams.
