Description Conduct evaluations for patients in both inpatient and outpatient environments and create customized treatment plans Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall function Work collaboratively with nurses, physicians, case managers, and rehabilitation professionals to optimize patient outcomes Guide patients and their families through therapeutic objectives, discharge planning, and home exercise routines Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with institutional and regulatory guidelines Engage in care team rounds and interdisciplinary meetings to ensure seamless care delivery Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program Certifications

State licensure as a

Physical Therapist CPR certification is preferred Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills

Previous experience is highly preferred Adept at treating patients with multi-faceted medical and mobility challenges Strong communication and collaborative skills Comfortable utilizing digital charting and clinical documentation platforms Benefits
