Program Overview:
Connect to Home is a supportive housing model working in collaboration with the Department of Children and Families Office of Housing (OOH) and the Department of Community Affairs to support youth and/or parenting youth who are currently involved with the Division of Child Protection and Permanency (DCP&P) or have been in DCP&P custody for at least one day in their life in Burlington County. These youth must be homeless or at risk of homelessness. These individuals are identified by the Burlington County DCP&P offices. This housing first program model supplements safe and stable housing for at-risk young adults with voluntary supportive services.
Position Summary:
The care manager is the primary contact for each program participant and has tremendous influence in shaping how each participant views the CTH intervention's services and benefits. Candidate qualifications must include a strong background in understanding and applying principles governing trauma-informed care, positive youth development, protective and promotive factors, harm reduction, and best practices in supportive housing.
As an integrated health organization Acenda values the use of evidence-based practice or EBP's.
Key Responsibilities:
Provide care management, outreach, engagement, and support to youth residing in supportive housing units. Ensures the safety, stability and well-being of youth and effectively work with and identify services for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs; Possess training and experience in trauma-informed care and integrated approaches to treatment and recovery from mental health and substance use disorders for youth and young adults. Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment. Provide individual support and counseling and assist with crisis management. Help youth secure public benefits, and develop recovery strategies. Train and build the competencies and skills of youth. Possess knowledge and competency in harm reduction, medication-assisted treatment, and recovery supports for substance use disorders, and exhibit knowledge of available treatment services and community supports. Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures. Performs other related duties as assigned. Requirements:
Bachelor's degree in social service-related field required, with a minimum of three years of experience working with children, youth, or families Must have and maintain a valid driver's license with an acceptable driving record Must be able to exercise independent thinking and good judgment under all circumstances. Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently. Additional Information
Base rate: $20.67/ hourly Bilingual Rate: An additional $1.50 added to the base rate if Spanish proficient. Sign-on Bonus: $500 to welcome you to our team! Schedule: Flexible Work Hours Job Type: Full-Time We Provide:
