Home Health RN - WLA Team at Kaiser Permanente Job Summary: As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.
Responsibilities
Demonstrates proficiency by exhibiting the following skills, competencies, and behaviors: Leadership
Upholds Kaiser Permanente’s Policies and Procedures, Principles of Responsibilities, and applicable state, federal and local laws.
Serves as a leader of the health care team; delegates tasks appropriately, and demonstrates accountability.
Understands own and team members’ scope of practice and escalates issues as appropriate.
Champions new ideas; leads and directs others through change; uses effective communication strategies including chain of command and issue escalation.
Mentors, orients, and coaches others in unit operations and patient care activities; shares responsibility and authority with subordinates.
Performs problem solving with other departments to resolve member issues; prioritizes, delegates, and supervises work to ensure completion of patient care activities.
Complies with regulatory requirements, policies, procedures, and standards of practice.
Nursing Process: Develops and contributes to the individualized plan of care; ensures multidisciplinary planning, consultation, and education; discusses the plan with patient and family, and completes it timely.
Monitors progress, revises the plan as needed, ensures care meets standards of practice, and completes discharge planning and education.
Ensures patient safety, pain control, understanding of medications, and discharge instructions; identifies barriers to outpatient medical management as needed.
Patient Care Experience: Adheres to customer service standards, promptly responds to call lights and patient requests, makes referrals, and maintains patient confidentiality; ensures a clean and functional work environment; communicates with families with courtesy, respect, kindness and compassion; delivers information clearly and ensures comprehension.
Team Commitment: Acts as a resource, preceptor, and mentor; participates in mandatory meetings and professional development; supports collaborative labor-management; contributes to unit learning and continuous improvement.
Fiscal Responsibility: Minimizes overtime and identifies system improvements; uses payroll and non-payroll resources effectively.
Experience
Minimum one (1) year recent full-time equivalent experience in a licensed Home Health, Palliative Care and/or Hospice agency as an RN/PHN (within the last three years).
Education
N/A
License, Certification, Registration
Registered Nurse License (California)
Driver's License (California)
Basic Life Support
Additional Requirements
Clean Driving Record for the last 2 years.
Proof of automobile insurance as required by law.
Notes
If twelve (12) hour option chosen, shift will be 8:30am to 7:00pm, back up 8 hour shift will be 8:30 am to 5:00 pm.
Weekend on rotation.
