Job Overview

The Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include administration of medications, documentation of medication administration, and providing resident care while promoting the Sunrise Mission: to champion the quality of life for all seniors, in accordance with applicable laws, standards, and Sunrise policies.

Responsibilities & Qualifications

Essential Duties Medication Administration: Provide high-quality care by administering medications and treatments safely and in an organized manner. Documentation: Review, read, notate, and initial the electronic health record to document resident information. Medication Updates: Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse. Medication Pass: Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse). Timing: Ensure medications are passed according to times using the mobile med cart. Documentation of Doses: Document and initial as medications are given; document refusals or missed doses. Confidentiality: Maintain confidentiality of all resident information including medications. Incidents & Stock: Report concerns and unavailable medications to the RCD or Wellness Nurse; restock medication cart after passes. Packaging & Assessment: Assist in checking medications and assess residents for as-needed medications, documenting and reporting to supervisor. Narcotics: Count narcotics with another MCM/LCM each shift and report discrepancies. Maintenance: Maintain and clean the Medication Room, carts, and treatment carts; manage supplies and expiration dates. Re-fill Process: Follow refill process and notify if a new prescription is needed. Independence & Dignity: Help residents maintain independence and safety, adhering to Sunrise Principles of Service. Empathy: Respond to residents with empathy, respecting communication patterns and needs. Care Precautions: Practice standard care precautions of cleanliness and hygiene. Resident Care Communications Notify the RCD of resident and/or family concerns. Attend daily Cross Over meetings and participate in ISP development. Risk Management & Safety Ensure compliance with regulations; promote risk management and safety. Practice PPE, fire safety, SDS, Lockout/Tagout; report accidents/incidents and hazards. Ensure safe storage of equipment and infection control techniques; handle blood-borne pathogens and hazardous materials appropriately. Report occupational exposures and monitor oxygen tanks as prescribed. Training & Team Contribution Participate as a team member and support engagement initiatives. Attend meetings and trainings; maintain required training compliance. Perform other duties as assigned. Core Competencies Decision-making in residents' best interest; communicate effectively; manage multiple priorities; demonstrate good judgment and problem-solving. Experience & Qualifications High School diploma/GED required; certifications maintained as required by state/provincial regulations (CPR/First Aid). Must be at least 18 years old; previous experience with seniors preferred. If driving a Sunrise vehicle, driver duties and safety responsibilities apply.
