Job Summary
The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include drug procurement, storage, safety, preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, clinical research, and public and clinical service. Under the Department of Pharmaceutical Services, the Operational Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System and may participate in pharmaceutical care in other sectors as needed. The Pharmacist II performs a wide range of pharmaceutical care functions to ensure drugs prescribed for and used by patients are safe and effective. These functions include obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on therapeutic plans, ensuring access to medications, educating patients on proper use and potential cautions, adverse reactions and interactions related to therapy. The Pharmacist II works as part of an interprofessional team including physicians, advanced practice providers, nurses, and other professionals. The role is based primarily in the inpatient pharmacy setting, managing specific patient populations if applicable. Depending on shift and site, the Pharmacist II may oversee pharmacy technicians to facilitate insurance authorizations for medications, in addition to preparing and dispensing prescription medications and pharmaceutical products for patient care. The Pharmacist II may also oversee clinical activities and delivery of services. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs. Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy Preferred Qualifications
Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of inpatient/operational pharmacy experience. Hold an advanced practice board certification (e.g., Board of Pharmacy Specialty certificates, AAHIVP, etc.) License/Certification
Must possess a current license in good standing to practice pharmacy in the State of California Current certification for Basic Life Support (BLS) from the American Heart Association About UCSF
