4 Session GP position with a mix of face to face, eConsults, home visits andphone call appointments in a friendly training practice in suburbanDurham. Working across three days, to include attendance at our weekly practice meeting Tuesdays. We are primarily seeking to recruit someone to take on the position of lead GP for Diabetes Management.

Main duties of the job
We are seeking an experienced General Practitioner to join our small, friendly team. We are offering a unique opportunity for a dedicated professional with at least 2 years\' experience in general practice and preferably with a diploma in Diabetes Management. The role carries considerable responsibility to ensure the highest standards of care for all our patients. The successful candidate will be involved in planning and decision making as part of the wider practice team. There is an expectation that the post-holder complies with all clinical and information governance policies and procedures and any other policies and procedures necessary for the safe and effective discharge of all required clinical duties. We take a proactive approach to looking after our patients and focus on continuity of care and innovationin service delivery We are a dedicated training practice, providing training and clinical support for 2 x GP Registrars and a Foundation L2 doctor. We also host 5th year medical students. We occasionally provide work experience for those considering GP as a career. Job responsibilities
To undertake all duties and responsibilities associated with a General Practitioner working within Primary Care including: Professional decision-making in relation to presenting problems. Take part in audit activities when required. Assessment of patient\'s healthcare needs Consultations face to face, via telephone and home visits and dealing eConsults. Triage calls and visits, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely manner. Proactive screening of patients for risk factors and early signs of illness. Disease management in line with protocols, developing care plans when required. Working together with the Nurse Practitioner as Diabetes Lead GP. Providing health education as necessary Attend and take part in MDT meetings with the wider team. Compliance with all relevant clinical governance and practice policies and protocols. Assisting with establishment of appropriate systems to manage long term conditions. Completion of all relevant administrative duties required for successful delivery of service. Record clear, detailed and timely consultation notes to agreed standards. Demonstrate commitment to lifelong learning and audit and continuous development to ensure evidence based best practice. Compile and issue acute and repeat prescriptions, prescribing in accordance with the formulary when appropriate. Meet professional learning standards and CPD requirements. Development of effective relationships with colleagues and the wider team which is very important for collaborative working. Supervision and support for trainees during their placement at the practice. Providing teaching and training session for trainees, as required. Person Specification

Evidence of annual appraisal and revalidation (where appropriate) General Practice trained Experience in Diabetic Management On Performers List Enhanced DBS (although this will be renewed on appointment) UK driving licence Current CPR certificate Evidence of continued professional development Other post graduate diplomas Experience / interest in teaching / training Diploma in Diabetes Management Experience
Excellent communication and interpersonal skills Flexible and punctual Motivated and a forward thinker Ability to process information accurately and effectively in a timely manner. Experience as Diabetes Lead GP High levels of integrity and loyalty Sensitive and empathetic in distressing situations Ability to work under pressure and in stressful situations Commitment to ongoing professional development Experience of working as a GP trainer.
