Hematology Nurse Navigator (Grant Funded)

Job Summary
The Hematology Nurse Navigator will assist and coordinate care of Hematology patients, with special emphasis on the transition of adolescent and young adult subjects with sickle cell disease to adult care. This position directly reports to the Principal Investigator.
Responsibilities
Assists in the care and coordination of Hematology patients.
Transitions adolescent and young adults with sickle cell disease patients to adult care.
Identifies unit system problems and takes steps to rectify same, in a cooperative effort with other health team members.
Exhibits courtesy and respect to all members of the health care team through teamwork and accountability.
Completes all documentation accurately.
Maintains a professional and positive attitude and provides consistent, exceptional customer service to internal and external personnel, patients and visitors.
Provides On-Call support for nights, weekends and/or holidays.
Collaborates with the work team to identify the needs of patients and families.
Maintains current knowledge in the area of responsibility.
Maintains license, certification or registration as required.
Ability to adapt to new and unusual situations or changes without affecting performance.
Performs job duties and assignments within appropriate time standards as determined by division.
Regularly reports to work when scheduled and returns from meal break within allotted time frame.
Maintains a professional appearance and neatness as outlined by APS guidelines.
Understands and performs job duties and responsibilities.
Actively participates as a team member by supporting decisions, accepting change, managing conflict effectively and valuing the contribution of others.
As an employee of APS, demonstrates professionalism through courteous, prompt patient, staff and vendor encounters, ensuring the values of the organization and commitment to quality patient care.
Supports and facilitates the grant funded research entitled "AYA SCD Transition and Nurse Navigator for Implementing Home Health Model".
Drafts the required Quarterly Report to NYS that is finalized by the Principal Investigator of this grant.
Contributes to required research relevant to this grant.
Performs other duties as requested.
Qualifications/Requirements Experience:
2 years previous Hematology, Oncology, Stem Cell Transplantation and/or Adolescent and Young Adult General Medicine experience, required.
Education:
Graduate from an accredited school of nursing, required.
Bachelors of Science in Nursing (BSN) degree, preferred.
Licenses / Certifications:
Active NYS Registered Nursing License, required.
BLS, required.
Other:
FDA approved automated CBC machine.
Computer, telephone system, copy machine, fax, paper shredder, calculator, and various standard office equipment and/or machines.
