SCHEDULE:

40 hours, Monday - Friday, 7 a.m.- 3:30 p.m. Pays $23.48 per hour DOE This position carries a retention payment of $3,000.00, $1,000.00 payable at 12 months, and $2,000.00 at 24 months Summary of the Position

The Case Manager will be responsible for the overall case management of the participants in the program at 28 Rockwell Street. This program will follow the models of Housing First/Safe Haven. The Case Manager will use harm-reduction, trauma-informed approach and Motivational Interviewing techniques to create and maintain a respectful atmosphere that empowers very chronic, mentally ill participants to reach their highest level of functioning. Participants will be assessed for their actual level of functioning in various spheres upon entering the program and assisted in individual plans to improve and enhance this level. The Case Manager is required to provide services to these individuals with the goals of housing stability and psychosocial rehabilitation, advocating for appropriate mental health services. Due to emergency or unforeseen program needs, staff may be temporarily or permanently reassigned to another Safe Haven program at any given time. Requirements

Education/Training

Required: Associate’s Degree (Can be substituted for a High School Diploma with at least 3 years of experience working in the human services field and/or in case management) Preferred:

Bachelor’s Degree in nursing or other related field such as counseling, psychosocial rehabilitation, or psychology A valid Massachusetts Driver’s License in good standing Advanced training in psychosocial rehabilitation and substance abuse Bi-lingual – Spanish/English Knowledge/Experience

Required: Minimum of three (3) years prior work experience in a therapeutic setting, community residence or in an inpatient setting Experience as a mental health team member Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment Experience conducting psychosocial assessments Experience with case management and implementing treatment plans Knowledge of treatments for chronic mental illness, including medications Previous experience working with dually diagnosed individuals Experience assisting people with ADL skills Experience working with other vendors and services within the public mental health system Previous experience moving homeless mentally ill participants into housing Preferred: Previous experience working with a Housing First/Safe Haven model Familiarity with the Metropolitan Boston public and private mental health system Long term treatment experience with chronically mentally ill people Seniority level
