Overview
Clinical Assessment RN - Homecare (Boston South). This in-home role provides clinical assessments for geriatric patients in residential settings, supporting care planning and collaboration with interdisciplinary teams to improve member outcomes. Responsibilities
Perform in-home assessments and clinical screenings using tools such as MDS, PHQ-9, and other standard instruments. Conduct medication reconciliation, assess adherence, and provide disease education. Escalate urgent concerns and refer members to appropriate services (e.g., behavioral health, LTSS). Document findings in electronic health records and care management platforms (EMR is ClinicalWorks). Support telehealth assessments when appropriate. Collaborate with care teams to develop individualized care plans and determine program placement. Schedule and Caseload
Schedule:
Monday–Friday, 8:30 AM – 5:00 PM (no weekends, holidays or on-call) Caseload:
3 assessments per day (15/week) Compensation and Benefits
Local: $60/hour Travel: $2,800/week Mileage Reimbursement: $0.70/mile Parking Reimbursed Benefits:
Competitive pay, weekly paychecks; health, dental, vision, and life insurance; 401(k) savings plan; awards and recognition programs. Benefit eligibility is dependent on employment status. Qualifications
Required
Active RN license in Massachusetts 1+ years of clinical experience (including clinical rotations) Experience with MDS (or Oasis) Strong clinical judgment, organizational and communication skills Proficiency in Microsoft Office and willingness to use telehealth tools Preferred
Bachelor’s Degree in Nursing Experience with electronic medical records Background in care management Bilingual in English and a second language About Amergis
