Information & Assistance Specialist (Bilingual English/Russian)
Under the general direction and supervision of the Senior Program Manager, the I&A Specialist will provide information and assistance, connecting adults and adults with disabilities and caregivers to programs and services, including public and privately funded options. Will assertively provide outreach activities in the form of booths and/or public speaking engagements for educational purposes on aging and disability services in the community. Works collaboratively with internal and external services in order to advocate for consumer needs to assure services are delivered.
Overview This position involves delivering information and referrals, community outreach, and case-management support to seniors and adults with disabilities, with a focus on languages English and Russian.
Responsibilities
Provide one-to-one assistance for seniors and adults with disabilities to enable them to resolve barriers.
Provide information and referrals to clients utilizing a wide array of resources, including but not limited to: adult day health care, caregiver assistance, community services, health and wellness, education, emergency preparedness, employment, financial assistance, food/nutrition, housing and shelter, in-home care, legal safety, recreation, senior centers, LGBT programs/services and transportation.
Provide up-to-date information and clarification regarding programs such as Medicare, Medicaid, SF CalFresh, and formal supportive and social services.
Provide translation assistance, verbally and in writing, to clients who cannot speak English, utilizing an interpreter as necessary.
Complete applications, review and respond to correspondence, fill out forms, and make or confirm appointments.
Conduct client needs assessments and evaluate needs, following up to ensure needs are met and documenting progress notes to ensure quality service delivery.
Maintain accurate and up-to-date access to resources.
Assist in planning and implementing a variety of special events and recreational activities for clients.
Complete accurate program and client data collection, data entry, and required programmatic reports on a timely basis.
Maintain confidentiality of all client records and comply with HIPAA privacy and security rules to the extent applicable.
Administer evaluation data collection and participate in program and agency performance management processes.
Create and submit monthly activity reports and monthly metrics in the CARBON system by the 15th of the following month.
Enter and submit required data into SF GetCare monthly.
Submit Community Services Block Grant time studies to HAS/DAAS in February, May, August, and November.
Develop and deliver ad hoc and annual summary reports of data collected (e.g., SOGI data) by specified deadlines.
Actively conduct outreach activities and recruit clients to ensure enrollment targets and ADRC Service Objectives are met.
Design and disseminate outreach materials and presentations to raise awareness of long-term service options.
Conduct outreach to a wide array of community members and organizations and develop partnerships with community agencies.
Provide exceptional customer service and represent the organization in a professional manner.
Maintain personal safety, use PPE, and follow organizational policies; report safety issues or injuries promptly.
Interact positively with team members and facilitate positive changes in collaboration with supervisors as needed.
Qualifications
Education:
A Bachelor’s Degree in Social Work, Human Services or related field; or an Associate’s degree in a related field or at least 3 years of social service delivery with elderly and/or adults with disabilities.
Experience:
Experience in interacting with seniors and disabled adults, case management, group facilitation, and recreational activities; commitment to social action, social justice, civic engagement, and leadership development.
Requirements:
Current Physical Health Screening (every 2 years), Fingerprinting Clearance (DOJ), Current Immunizations (TB, Influenza, MMR, Pertussis), CPR & First Aid (every 2 years), Valid California Driver’s License with current auto insurance and a clean driving record.
Language:
English/Russian bilingual.
Skills & Abilities:
Independent thinking, strong customer service, integrity, multitasking, effective communication, and ability to work in a multicultural team; strong analytical and organizational skills.
Software:
Proficiency in Microsoft Office: Outlook, Word, Excel, PowerPoint, Publisher; general computer proficiency.
Decision Making:
Set priorities and develop plans to accomplish work.
Tools & Equipment:
Standard office equipment; able to use a computer, fax, copier, and phone.
Confidentiality:
Maintain the highest level of confidentiality with client records.
