Essential Functions & Responsibilities

The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for. Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising facility clinical guidelines; ensuring compliance with state and federal regulations; training facility staff; and supervising facility clinical staff. As attending physician for residents at each facility, the physician collaborates with Nurse Practitioners and Physician Assistants to manage day-to-day resident care. Each facility typically has a full-time PA or NP assigned based on acuity and patient volume. Provide high-quality, patient-centered, comprehensive primary care services by rounding on SNF residents. Diagnose and treat acute illnesses (e.g., bacterial/viral infections) and chronic diseases (e.g., hypertension, heart disease, diabetes). Document in Theoria’s EHR system, ChartEasy; participate in other EHR systems used (PointClickCare, MatrixCare, American HealthTech). Participate in Theoria’s Chronic Care Management, Transitional Care, RPM and other quality initiatives and programs. Supervise collaborating nurse practitioners and physician assistants. Utilize electronic/online medical tools and state/county systems for administrative tasks such as Death Certificates.

Shift Structure

Shifts are flexible depending on physicians’ schedules. Typically, medical directors work a minimum of 2-3 days/week at each facility they oversee. Rounding times are flexible with no set start and end time. Start preferably between 7–10 a.m. and finish when caseload is seen. Call is covered by Theoria's virtual care team. This is an in-person position.

Qualifications

Licensed to practice medicine in the state(s) of operation. Experience in medical directorship or SNF primary care preferred. Ability to supervise NP/PA teams and use EHR systems (ChartEasy, PointClickCare, MatrixCare, American HealthTech). Willingness to travel as needed.
