Overview
Full Time Physical Therapist Case Manager — Home Health. Anticipated Territory: Multiple Opportunities, Small Coverage areas in SF. All opportunities require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted.
Compensation Base pay range: $120,000.00/yr - $135,000.00/yr
Wage range: Physical Therapist, Level 1: 120,000.00- 135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match). As a practice, the majority of PT Case Managers are hired as PTIs. Once it is determined that an PTI can supervise 2 PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to an PTII with a minimum salary of 135,000 plus 20K bonus (Paid Quarterly).
What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan and work with PTA's to follow the care plan.
Help patients with mobility, movement, and helping in regaining their independence.
Create detailed treatment and care plans for each patient.
Communicate and coordinate with patients’ physicians to ensure a quality level of care.
Evaluate and report patients’ progress to MD.
What you’ll need to be successful
Current unrestricted CA Physical Therapist License
Current BLS/CPR Certification
Valid CA Driver's License
Knowledge of local community resources is required.
Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
Knowledge of state and federal homecare regulatory guidelines preferred.
Experience home health care is highly preferred
Experience with high acuity patients is a plus
Experience working with an interdisciplinary team is highly desired
Case Management fundamentals is highly desired
