Overview
Full-time Physical Therapist Case Manager, Home Health, in the Pacifica area. Anticipated territory: Pacifica. In partnership with other clinicians, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively in their home environment. Base pay range: $120,000.00/yr - $135,000.00/yr. Additional compensation details include potential bonuses as described below. All opportunities require field visits to patients in their homes; remote/work-from-home positions are not available unless noted. Responsibilities
Perform an in-home evaluation and develop a care plan; work with Physical Therapy Assistants (PTAs) to implement the plan. Assist patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a high level of care. Evaluate and report patients’ progress to the physician. What you’ll do as a Home Health Physical Therapist Case Manager
Provide skilled services to evaluate and address patient needs in the home to promote safe and productive living. Evaluate and treat patients using medically prescribed physical therapy programs. Supervise PTAs per agency and regulatory standards. Other responsibilities as assigned. Qualifications
Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Understanding of development, human behavior related to illness/injury, and the influence of culture on health care Knowledge of state and federal homecare regulatory guidelines preferred Experience in home health care highly preferred Experience with high-acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Self-directed, empathetic, critical thinker with outstanding interpersonal skills and leadership abilities Organized Benefits and compensation
