Responsible for medical care of HCHP patients at the health centers and the supervision of physicians and Nurse Practitioners in the center. Supports and facilitates interdisciplinary team approach to health care delivery and monitoring of quality. Provides leadership in creating a culture of collaboration and excellence in clinical care, education, research, and evidence-based practices. Promotes continuous performance improvements in clinical and health system practices. Ensures organizational compliance with regulatory and accreditation requirements. Responsibilities

Oversee medical care for HCHP patients at health centers and supervise physicians and nurse practitioners. Promote interdisciplinary team collaboration in health care delivery and quality monitoring. Provide leadership to foster a culture of collaboration, clinical excellence, education, research, and evidence-based practice. Lead continuous performance improvement initiatives in clinical and health system practices. Ensure compliance with regulatory and accreditation requirements. Qualifications

Degrees: MD/DO required. Licenses & Certifications: Family Medicine MD (or) Internal Medicine MD (or) Pediatrics General MD; Physician must be licensed/registered in the State of Texas. Work Experience: Three (3) years of experience with patient navigation, care management, or direct patient service in a health care setting. Language Skills: Bilingual preferred; Spanish/English; English/Vietnamese preferred. Communication Skills: Above average verbal (heavy public contact), writing/correspondence. Proficiencies: PC literacy. Knowledge/Skills/Abilities: Analytical. Work Schedule: Flexible; eligible for telecommute; travel; weekends. Other Requirements: Access to reliable transportation to travel to worksites throughout Harris County; passion for the community; self-starter and eager to learn, improve, and grow.
