Job Summary Responsible for medical care of HCHP patients at the health centers and the supervision of physicians and Nurse Practitioners in the center. Supports and facilitates interdisciplinary team approach to health care delivery and monitoring of quality. Provides leadership in creating a culture of collaboration and excellence in clinical care, education, research, and evidence-based practices. Promotes continuous performance improvements in clinical and health system practices. Ensures organizational compliance with regulatory and accreditation requirements.

Qualifications

Degrees: MD/DO required.

Licenses & Certifications: Family Medicine MD (Or) Internal Medicine MD (Or) Pediatrics General MD; Physician must be registered in the State of Texas.

Work Experience: Three (3) years work experience with patient navigation, care management, or any kind of direct service provision to patients in a health care setting.

Language Skills: Bilingual; Spanish/English; English/Vietnamese preferred.

Communication Skills: Above Average Verbal (Heavy Public Contract), Writing/Correspondence.

Proficiencies: PC proficiency.
